During your confidential virtual consultation, you will have the opportunity to meet with Dr. John Sarbak to discuss your concerns, review photos of the areas you’d like to improve, learn about treatment options, and get answers about pricing. A virtual consultation is your chance to learn as much as you can about Dr. Sarbak, our practice, and how we can best help you on your self-improvement journey!

How to Schedule Your Virtual Consultation

To schedule your virtual consultation, please fill out the form below with your information and a member of our team will connect with you as soon as possible to set up your appointment. Next, you will be asked to fill out a medical intake form and submit photos through our secure, HIPAA-compliant TouchMD platform. An email from our practice will outline the process for downloading the MyTouchMD app and creating your profile (if you do not already have one with us). All required paperwork and photos should be submitted at least 48-hours prior to your appointment. Taking Photos for Your Virtual Consultation

Properly photographing images of the areas you want to discuss with Dr. Sarbak is a very important part of having a successful virtual consultation. Our team will send you a helpful guide (with corresponding visuals) that outlines how to take digital pictures of specific regions of your body and/or face and submit them via TouchMD.
